Basic
Qualifications
Education/Training: A high school diploma or
equivalent; successful completion of an Indiana state-approved Nursing Assistant
Certification course or other education that meets state certification
requirements. Must be eligible to obtain certification within 120 days of
employment.
Skill(s): Moderate reading, writing,
grammar, and mathematics skills; proficient interpersonal relations and
communicative skills; auditory and visual skills.
General
Responsibilities:
Provide quality nursing care
to residents, follow specific procedures, policies and programs (Infection
Control, Resident Rights, facility policies and procedures); report pertinent
information to the immediate supervisor; assist immediate supervisor with tasks
to support department operations.

Safety, including universal
precautions and safe work practices, established fire/safety/disaster plans,
risk management, and security, report and/or correct unsafe working conditions,
equipment repair and maintenance needs.
b.
Confidentiality of all data,
including resident, employee and operations data.
c.
Quality Assurance and
compliance with all regulatory requirements.
d.
Compliance with current law
and policy to provide a work environment free from sexual harassment and all
illegal and discriminatory behavior.
- Supports and participates in common teamwork
a.
Cooperates and works
together with all co-workers; plan and complete job duties with minimal
supervisory direction, including appropriate judgment.
b.
Uses tactful, appropriate
communications in sensitive and emotional situations.
c.
Follows up as appropriate
with supervisor, co-workers or residents regarding reported complaints, problems
and concerns.
d.
Promotes positive public
relations with residents, family members and guests.
e.
Completes requirements for
in-service training, acceptable attendance, uniform and dress code including
personal hygiene and other work duties as assigned.
Essential
Duties
- Answer resident’s signal or call light promptly.
- Perform personal care as needed:
- Bed bath, tub or shower (as per schedule)
- Oral care each shift, hair and nail care, shaving, dressing and undressing, application of make-up.
- Ambulating, transferring, positioning by using assistive devices such as a walker, wheelchair, Sara lift, Hoyer lift, gait belt or lift sheet.
- Encourage fluids and chart the intake and output of resident.
- Assure that water pitchers are clean and filled with fresh water daily three times a day.
- Prepare residents for meals.
Serve meal
tray. Feed or cut food as needed. Assure that resident’s food is
accessible.
Feed
residents who cannot feed themselves. Remove tray after resident has
finished.
Clean
resident’s hands, face, clothing, etc.Chart required information as
instructed.
- Notify supervisor of anything in the resident’s condition that calls for attention.
- Wash out soiled linens.
- Make bed and change linens as needed.
- Assist resident in obtaining their optimal level of physical activity.
- Maintain CONFIDENTIALITY of all pertinent resident care information to assure resident rights are protected.
- Attend all required in-services.
- Other related duties as directed by supervisor.
- Treat residents and other staff members in a respectful and courteous manner.
- Follow the instructions of the C.N.A. assignment sheet for specific care requirements for each resident.

skills necessary to safely and efficiently operate the following equipment and
machines used in the performance of this job: Telephone, Mechanical lifts,
Electronic scales, Adjustable chairs/beds, Various other equipment and
supplies
